Do they have homecare services?

Yes, but home care can take many forms – from nursing care to personal support [attendant care]. In order to determine what services are right for you, the CCAC will set up an assessment with a Care Coordinator. Call your local CCAC to make a referral and we'll get you started down the right path.

How do I get started?

To become a patient of the CCAC, you must participate in an initial assessment to help evaluate your needs and determine whether CCAC or other services are right for you. To get started, we just need a referral. You can refer yourself, or a referral can be made by your doctor, nurse, community service provider or other health-care professional. A referral can also be made by a family member, a friend or a neighbour. A simple call to your local CCAC gets the process started.

How long is the wait list?

In 2012/2013, CCACs ensured:

50 per cent of patients referred through the community receive home care service within 5 days and 90 per cent of patients receive service within 33 days

50 per cent of patients referred through hospital get home care service within 1 day of referral and 90 per cent of patients receive service within 6 days.
